Hysteria isn't a fucking thing

ok fun fact: I'm rapidly becoming a cult favorite doctor among our local privileged elderly white ladies, which I have mixed feelings about, but the #1 reason is that I just don't leap to "anxiety" as an explanation for symptoms unless the patient tells me "I am anxious, and then I feel these symptoms, and when I am not anxious, I don't feel these symptoms."

The sheer number of women I've seen who've been told for years to decades that the only thing wrong with them is anxiety is fucking staggering, in this Year Of Our Lord 2023, and I just keep digging. We checked a basic lab panel, sure. CBC. No anemia. CMP. Kidneys are fine. (Electrolytes are basically always going to be fine if someone is well enough to walk into my office under their own power to talk to me. Exception is mild chronic hyponatremia.) And we check thyroid. TSH and free T4. We check blood sugar. A1c, if the fasting is a little weird. Fasting insulin, if I'm still suspicious. We check cortisol. Inflammatory markers--ESR and CRP.

And eventually, if the symptoms support it, or right away, depending on my level of suspicion, we check rheumatological labs for abnormal autoimmune function. Anti-nuclear antibody. Rheumatoid factor. There's at least a dozen you can check, and which ones you should check is always a matter of debate and also of expertise that I 100% lack. We are out in the sticks. There are no "local" rheumatologists for me to send people to.

But a couple of weeks ago I found a woman--she has bipolar disorder and has been told for decades that's all that's wrong with her--who has an anti-centromere antibody titer that's fucking through the roof. I found an anxious 19-year-old with an ANA of 1:1380. And yesterday I found out why a sweet elderly woman I've seen for a year or two now started feeling crappy months ago: her rheumatoid factor is over 90.

Rheumatological disorders are always difficult. Our understanding of them varies from "pretty good, actually, and here are useful treatments" to "Well I Guess That Exists." Labs aren't always a slam-dunk and even labs plus symptoms can give you misleading impressions. Your immune system can decide that virtually any short chunk of protein is an enemy, and the problem with that is that your body is made up of many, many, many short chunks of proteins, so the odds that you'll develop some kind of antibody against yourself just keeps going up over your lifetime. Immune disorders tend to travel in packs; there's a clear genetic element to it, so the more first-degree relatives (parent, sibling, child) you have with any kind of autoimmune disorder (including Type 1 diabetes), the higher your risk of any kind of autoimmune disorder is, and if you already have one autoimmune disorder, you're at higher risk for developing another one.

But I think it's precisely because they're difficult that a lot of mainstream primary care prefers to pretend they don't exist, rather than try to sift through the utter fucking mess that is Mixed Connective Tissue Disorders, a title that has fallen out of favor since I learned it in my third year of med school. And women are at higher risk for autoimmune disorders than men. And older women are at higher risk than younger women.

So if I, as a family doc, just keep digging, just keep poking at the tangled knot of symptoms, there's a decent chance I will uncover something interesting. Hopefully something treatable. Sometimes we have nothing to treat with, and I just get to offer someone more understanding of their disorder, which feels pretty paltry but is better than the casual dismissal of "You're just anxious."

Never, ever, ever take anxiety as a diagnosis for a symptom other than anxiety. Not even as a rule-out. Keep those symptoms as an open question mark on the patient. Don't say "anxiety" just so you can close the door. And damn sure don't do it to women.

So i got a thought of insomniac Reader who may as well stay awake for who knows how long being somewhat terrified of sleeping.

He may be plagued by nightmares of some kind, having a fear of not simply waking up anymore or possibly just too many thought that run rampant and he can't keep them still enough.

Just lying in bed with both the men as they either sleep or one or both are still awake and try to help him. But they have a little time window sometimes which means Reader may be alone most of the time and left with his own thought more than they'd like to let him.

Thinking how he'll greet them when they wake up to a case and come back just working as normally as he can manage both of them seeing how the lack of sleep is dripping off if the man.

I think both of them be dragging the poor man to bed one at a time if he starts wandering bc he's restless as fuck.

The one time they actually hear about him sleeping when on a case is when Jessica, Hotch's Sister-in-law (is it ex?-sister-in-law bc Hayley's dead? Idk may be), comes to pick up Jack.

The silence in the house was uncanny, it felt displaced as Jack wandered around the house he called home. It was usual for him to see one of his dad's wondering around mumbling to himself about something he never could catch as Aaron and Derek had left for another case.

The boy knew it wasn't exactly normal for him to be in a house of silence. Wandering to the master bedroom door, he slowly and lightly opened the door with a creek.

It didn't do anything to alert the man inside as all the boy heard were soft snores. Otherwise the room was completely silent, sun pouring in and dappling spots on the bed and the covers where he could see a imprint of a person splayed on the bed, spotting strands of (h/c) hair on the head that was slightly tucked under the covers.

The morning was silent as Jessica opened the door, looking around lightly and calling out to Jack and Reader. Nothing came of it as no answer was heard, wandering inside the house for a little bit she found the master bedroom's door ajar.

The view of seeing her nephew hugging a body of one of the lovers of Aaron was adorable, just laying besides him and making a shushing motion towards her as she called out Jack's name. The boy understood that R had difficulties sleeping and he didn't want him to wake up yet, it was unknown go R if that was the reason Jack staid with him or not. Just guarding, bc he had heard Jessica tell about a fly that the boy was trying to slap out of the air when it came too close at least once.

Except that yet turned into sleeping three days with R regretting everything. At least he could have been blessed with some pictures of the scene or a possible stuffed animal or two besides him bc Jack didn't want him to be alone when he woke up.

I really think the boys all of them would do something small for R to try to help him. Doing subtly things that would not irritate him if he was known to be really easy to be irritated when lacking a lot of sleep.

Jack would absolutely share his studies with him, saying they'd scare off the nightmares or thoughts if R had ever answered a question from the boy on why he was up. Not that R would take them, he'd give them back at the end of the day saying it's important that they're protecting Jack instead of him and stressing about it to the boy. But he does not hear it.

I think R would be doing things out of boredom at night when he's restless and doesn't feel comfortable cuddling with Aaron and Derek for the moment. Were it baking, maybe crocheting, crafting or doing something else if R is handy enough and there's something to fix he could do it, he would do it to pass time.

I feel like I should make some things VERY clear.

-I am anti diet culture. I don't believe in restricting whole foods groups (with exception for disability/health/allergies/intolerances/moral reasons. I don't believe in fad diets. They're not only dangerous but also ultimately useless.

-I am anti body shaming and that includes being anti fat shaming AND anti skinny shaming. I'm against all body shaming 100%.

-I believe people can eat whatever they want and have whatever lifestyle they want without outside judgement. I do think legitimate concern from *close* family and friends may be acceptable at times. If you're undereating to the point of starvation or overeating to the point of causing deadly health issues then I think it should be considered okay for close friends and family to bring it up.

-I believe doctors do need to stop taking the easy way out for diagnosing folk. Illnesses and pain should not be automatically be assumed to be caused by weight loss or anxiety. I also believe that it is okay for a doctor to bring up weight if they are legitimately concerned for specific health reasons such as fatty liver, type 2 diabetes, hypertension, etc.

-I don't believe fat people existing is promoting obesity and fat people don't need strangers to comment on their body and/or health.

-I believe discussions about the difficulty of healthy eating and the issues the standard american diet is creating over the years can be done with out scare tactics. I don't believe documentaries about health need to do a bunch of dehumanizing headless close up shots of fat people to basically scare people.

-I believe fat is a neutral descriptor. I am fat, nothing is morally wrong with me or wrong with anyone else on the basis of fatness.

-Any pro-ana, pro-ed (including pro-BED) blogs that comment, like, or reply to any of my posts are automatically blocked.

-I'm not putting up with misinformation and lies about health

-I need abled bodied people to know that calling disabled people (specifically physically disabled people) "marginalized bodies" is both ableist and dehumanizing.

"dazies" by yeule

Layered and allusive, "dazies" is one of the densest songs - on one of the most intense and elaborate albums - that I heard all year. It's a song concerned with trans identity and physicality and cannibalism (among other things.) These are, coincidentally and unintentionally, qualities it shares with last year's #1 song, Ethel Cain's "American Teenager", something that only registered for me about halfway through writing this year's posts but which is impossible to ignore now that I have. What is going on with transness and cannibalism in the 2020s? And what does it mean that these two artists - not frequently mentioned together - have both made that connection and both delivered the most unforgettable visions of the past two years? I am almost certainly not the person most qualified to answer these questions! But since as far as I can tell I'm the only one asking them, here we go: I would say that the current ascension of fascism and accompanying anti-trans movement has created a sort of pressure cooker environment that's intensifying connections that might otherwise be subtextual. So we're seeing similar themes bubble up more frequently than they otherwise might; Ethel Cain joked, "Funny, I never considered myself tough" and now yeule coos, "Violently biting off the flesh/ Of your own/ Of your own/ Body."

This is not exactly the same, to be clear: one of these characters has been murdered and the other is eating themselves alive. Both are fucked but consent and bodily autonomy only apply to the second. Conceptually, though, they're operating on the same frequency, delving in the same vein of ore. They're about the body as fuel, as substance and sustenance. And to be fair, all humans are worm food in the end! But transness - the sense that on whatever level our physical or perceived self isn't right - puts this fact on the surface; trans people are both the worms and the wormfood, consuming the self to become it. And this must happen within an era where the world - and more specifically, the people with power - hate trans people, and where trans people must try to simply exist knowing this fact, hating that they know it, and either letting it consume them or consuming themselves first. Violence is a form of control, and the threat of violence is one which trans people must carve out an existence under, and violence toward the self is one of the last refuges when nothing else feels controllable. (This feels uncomfortably pro-self-harm or pro-ana so let me caveat that there is a difference between bad things and good ones, and trans affirmation is firmly in the latter category.)

The self must perish so that it can be remade, and as we know, destruction is just another form of creation. So if consumption is destruction then self-consumption is simply self-creation. This post is starting to both spiral into itself and get away from me, so let's consider it a rough draft to be rewritten if this theme continues to find a home in future songs; destruction and creation never end; the point is that "daizies" was the richest and most rewarding and enveloping song I heard all year. Anyway. When yeule sings, "Violently biting off the flesh of your own/ Of your own/ Body" I'm pretty sure they mean something like all of this. And when those guitars howl and shriek I'm pretty sure that's what it feels like, too.

"dazies" (and all of tortured, beautiful softscars) is best homage to the Smashing Pumpkins we got this year, maybe ever! As someone who grew up relating hard to the original, what made the Pumpkins (and Billy, specifically -- I won’t apologize for him except to say that everything he does, everything, makes sense when you read the story of his life) so special was that they were openly hurting. It wasn’t under a slick veneer, it wasn’t intellectual, it wasn’t cool at all. It was just sobbing and screaming at the same time, and often there was a lot of prettiness in this painful offering. It's what made them acutely embarrassing, as well. I bought and sold Siamese Dream more times than I can remember; as soon as I accepted that I liked that record and that, yeah, I felt those ways and did those things and always cried to “Mayonnaise” I immediately felt a whiplash effect of self-rejection. Those were bad feelings, bad things, stop crying about wanting to be you. And it goes on! That psychic rubble falls to dust in the background of my mind but I no longer pretend to be someone who doesn’t love Siamese Dream. And I think yeule, and the broader acceptance and promotion of yeule as, like, not a joke, not something where we say “despite all my rage I am still just a rat in a cage HA HA HA,” you know, laughing at someone’s pain, suggests that no one has to pretend to not love Siamese Dream anymore. I think that’s great, that’s really beautiful. I hope that wave of acceptance extends and swells and begins to encompass the meaningful and not just the petty differences of taste and opinion.

yeule sings of “cries and cries and cries,” of hurting their body as a proxy for joy, of their “sick heart, sick mind” and it feels like some cycle is complete. The ouroboros is also an example of self-consumption and completed cycles, but this particular ouroboros feels more like running out of thread in a weaving, another piece is carefully attached and the project continues. The snake of unbearable sadness coils atop itself instead of taking its tail in its mouth. The old stories end and the new ones are joined to their endings through recognition of ourselves in the other. Today the kids can take comfort in yeule, a more perfect avatar for not being ok.

there needs to be a game that does the thing overwatch did with ana except good this time

and by that I mean they need to make the sniper like. a 20mm anti-materiel rifle or a rotary autocannon (both using APHE rounds) or a double-barreled 8 gauge shotgun/60mm canister round cannon instead of a dinky little dart gun

and it needs to sound like the sky breaking open and hit like a bullet train (does massive damage to enemies and throws teammates across the map and blinds and stuns them)

would this be balanced? no. would it be good game design? also no. would the character be remotely viable? probably not

but the idea of being at 1 hp and getting flashbanged for three seconds and having the entire game's soundscape just being overpowered by a massive boom then waking up on the other side of the map at full health is hilarious to me

(would it be funnier if you could wallsplat teammates with it if they hit a wall too fast/early/late or would that make the character unplayable?)

also I guess to balance it against enemies so you don't get a healer who can solo wipe the enemy team, give the gun like. war thunder aiming where you have two reticles, one that's your aim and the other that's where the gun is pointing, and the gun aiming vector is on a decent delay so the healer has to wait for it to catch up to where they're looking if they turn too fast

and also like. I dunno. titanfall kraber velocity and bullet drop
