"Clarke, no." Lexa cannot help the halfhearted, exasperated smile that pulls at the side of her mouth as her wide-eyed girlfriend inches forward tentatively with what looks like a very fuzzy lump of coal in her hands.
Clarke gestures wildly in protest at what she knows will be a defensive argument by her botanically inclined girlfriend, clearly going on the attack to prove her case as the teeny black kitten sails within an inch of Lexa's nose. Lexa goes cross eyed trying to examine the animal as she refocuses, Clarke now cradling the baby protectively into her chest as the kitten meows in protest at the roller-coaster treatment.
"Someone left her all alone in a box outside the studio, Lex. A box-! She has no siblings, nothing, just a tattered little towel-"
Lexa raises a hand halfheartedly to cut off Clarke's indignant rant that she senses gathering like thunderclouds behind stormy blue eyes. She pinches the bridge of her nose halfheartedly to ward off the stress headache that she could feel brewing behind her temple.
She had known it would be something like this when instead of the series of excited voice memos and anecdotes that usually blew up her phone after Clarke had excited her studio for the day, a solitary text with a You home, Lex? lighting up her phone as the sun started its graceful meander below the horizon. Lexa, having loved Clarke almost as long as she'd known her, had signed and poured herself a glass of whisky and settled in to wait for her impulsive girlfriend to arrive home, knowing that she had some unknown in store for her.
Which, as fate would have it, would turn out to be an art studio parking lot kitten.
A neon green gaze peers out from between Clarke's parted fingers, the little ball of soot that Clarke has now named Hiro drinking Lexa in with a keen gaze.
Clarke, sensing that the kitten was having its non-verbal say, wisely held her tongue as Lexa and Hiro stared at each other for a long moment.
Prrt?
Hiro inquires softy at Lexa, the soft chirp punctuated by an inky paw waving halfheartedly through the gap in Clarke's fingers in Lexa's general direction as the small creature becomes slightly more inquisitive.
A long, graceful finger bridges the gap to stroke the fluffy black fur. Clarke fidgets slightly but stays silent as bright blue eyes flick excitedly between Lexa's thoughtful hand and Hiro's tiny frame.
Silence stretches on for a long moment as Lexa softly dances her fingers across the rim of the whisky glass, thinking.
A rusty, quiet purring breaks the moment as Hiro arches into Clarke's hands, rubbing her slender face against Clarke's palm.
Lexa follows the interaction quietly with narrowed eyes, sighing as she drains the last of her whisky.
"Fine, bring her in. But she gets a bath first thing, do you hear me, Clarke? And she's going to have to learn not to chomp any of my plants- do you hear that, Hiro? You have to be good, do you hear-"
Clarke, grinning from ear to ear, sails past Lexa with her miniature void in hand, Hiro rumbling contentedly as Clarke smacks a grateful kiss onto Lexa's cheek before running inside.
Clarke pretends not to notice when Lexa starts snuggling Hiro after long days at the shop, Hiro tucking up into Lexa's lap contentedly while she reads, small paws moving neatly as she kneads Lexa's thigh.

Have you ever thought of moving abroad? If you could live anywhere, where would it be?
I have thought about moving abroad! It was all I wanted to do when I was younger (and we had no money and couldn't afford to go anywhere overseas). Back then I thought a lot about Canada mostly because it was cold, or New Zealand, because it was cold, lmao.
When I was able to travel, I thought about Melbourne (which isn't really moving abroad, but hey, it is a 4 hour flight away which feels like the same thing sometimes), and also Scotland, which I well and truly fell in love with. We even priced up real estate in Scotland and talked about making it feasible.
But...unfortunately, having an extremely rare genetic and incurable cancer condition means you almost always have to be very close to a hospital with specialists who actually understand your condition and manage it, and Scotland actually isn't known for being one of the best hubs in the world for the SDHD gene fault. On top of that, the NHS wouldn't be free for me as healthcare, and the yearly MRIs, thrice yearly PET scans and blood tests alone to monitor things are a lot of money if you're not able to access socialised healthcare.
There's only a few places in the world that would actually be kind of skilled enough and have the right kind of specialists for my disease, and that ruled out New Zealand, Scotland (especially the time spent waiting to access the NHS in the first place), Canada, and anywhere else I wanted to live. It's left us with other locations in Australia, but again, the only other place I really want to move is Tasmania, and anyone who wants treatment for this condition in Tasmania has to fly to Melbourne and back again just for consultation appointments.
To give you an idea of how hard it is to find specialists.
I am...miraculously lucky here in Perth. I almost thought we'd have to leave the state for a few months to find a specialist for my condition, and it turned out we lucked into two, and I have one of the best in Australia. Depending on the number of tumours I grow and what they're doing, I need access to the best neurosurgeons, vascular surgeons, endocrinologists, ENT surgeons, and medical and radiation oncologists who know what PRRT is, or gamma ray, or debulking. People with this condition die in places with specialists who don't understand. These tumours can cause a hypertensive crisis if you so much as touch them - biopsies have literally killed us, and many don't understand not to do them because when most doctors see a tumour they want to stick a needle in it.
I don't recommend having disabilities of certain kinds - particularly the kinds that need unique management (one of my tumours occurs in only 1/20,000,000 people to give you an idea of how rare it is) - if you have dreams of moving abroad. It was a pretty sad realisation honestly, because I'd be like 'but what if- oh, the tumours.'
These ticking time bombs basically mean I can never move far away from a hospital of any kind, and it's incurable, so you know, that's that pretty much done. No more indulging thoughts of moving abroad. But I'd like to visit Scotland again one day, when it's safe enough for immunocompromised people to travel freely. Which will probably not be in my lifetime. :/

I’ll have what she’s having.
Yesterday we had a consult with Dr. Ursina Teitelbaum at Penn. Aside from the issue of it taking an hour and a half to get there and then not being able to find the Abramson building or the correct parking garage, the Penn experience was a good one. (We get it, Abramson family, you’re super rich and love donating buildings). First, we met with Dr. Scully, who reviewed my history and diagnosis, and then she brought Dr. Teitelbaum in.
Dr. Teitelbaum started the appointment by saying while she didn’t have a plan yet, she hopes to have one by Friday. There is a monthly meeting at Penn where the cancer team gets together and discusses things and I happen to be one of those things tomorrow. She said there are three head honchos, who oversee the three main divisions of the cancer center. One, the head of “weird cancers” will be helping her work on a plan. She introduced what she called a "menu" of treatment options. It was not a menu in the traditional sense, where I could choose an entree, but more like a if/then flow chart with lots of catch-22 situations mixed in.
One of the biggest things I took away from this appointment was that I am a medical quandary to doctors, not because of the unusual nature of having two cancers, or even the rarity of my current cancer, but rather that I was young when I received the treatment for Hodgkins, and that chemotherapy has a lasting effect on my body, one that will unfortunately alter the plan for treatment now. Since I'm young and otherwise healthy, the "best" course of action would be to aggressively treat my liver, in hopes to remove the tumors or at least place them into a "hibernation." The issue is that all of the aggressive forms of treatment lead to future cancer, and being 37 years old, means it could certainly have negative implications before I am too old for it to matter. The primary concern is bone marrow cancer.
When I bluntly asked about my prognosis, or life expectancy, Dr. Teitelbaum said she REALLY didn’t like what she saw in my liver. She said there is a lot of unknown since many of the treatments available now didn’t even exist the first time I had this cancer, and so the future may provide more options, and my prognosis may be an ever changing thing, versus something definitive. This is hard. While I don’t want to hear that I have five years to live (or something relatively as morbid) it’s nice to know, and we just don’t.
Regarding treatment, everyone agrees, the first step is the Lanreotide. It's a simple decision because it has proven to be effective in many cases, it has minor side effects, and few long term effects on the body. Although, for the first time ever - Dr. Teitelbaum mentioned Lanreotide causing hair loss. Fun.
She mentioned debulking surgeries (taking large portions of the liver in hopes it will grow back healthy - sounds like multiple resection surgeries) but it’s a pretty intense process with the possibility that the cancer comes back. So onto other options...
Apparently there are three types of embolizations possible. These involve a tube going into the femoral artery in the groin, snaking it through the body to the tumor, and shooting beads into it to stop the blood supply and thus rendering the tumor inactive. The first, which is what we’ve always discussed, is bland embolization. That, is the most simple (or maybe blandest, bu-dum-dum) version. Then there is chemoembolization and radioembolization - which, to the best of my knowledge, is the same thing PLUS chemo or radiation in that targeted area. This would be more effective and also more dangerous (because of long term effects).
One of the other treatment options, based on the amount of cancer going on in my liver is Peptide Receptor Radionuclide Therapy or PRRT, which is a newer treatment, that was happening in Europe more often than the states (people were actually going to Europe to receive this therapy when it was not happening here) and it was historically reserved to treat tumors that were not receptive to somatostatin drugs alone, like Lanreotide, as a next step. Recently, according to Teitelbaum, it has been use as a more preventative (or earlier) measure as well. This treatment involves a peptide (cell-targeting protein) combined with a small amount of radiation to be injected into the bloodstream. The radiopeptide travels to and binds to the neuroendocrine tumor cells and delivers a high dose of radiation, thus "killing" the tumors. I don't know anything about the side effects of this, other than exhaustion and I have no idea how or when it would occur and how it would interfere with general life things.
The final additional treatment options were oral chemotherapy and targeted oral treatment. Both of these have more negative side effects and possible long term implications.
Lastly, the team wants me to do some genetic counseling. I have a second cousin with a similar condition (pancreatic and liver NETs) and my grandfather also had tumors - so it’s worth checking out. And this is something we can get ahead of with the kids if it turns out to be genetic after all. I can’t even begin to think about that - but I do need to know.
Dr. Teitelbaum recommends getting scanned every three months - probably an MRI, rather than a PET or CT scan to reduce the amount of radiation I am exposed to. This should be enough to monitor the liver and ensure we are making smart decisions about treatment.
I hope to have some more information tomorrow and will update accordingly! I was told my ears may be ringing between 7:30-8:30 a.m. — so I hope I can sleep!

Peptide receptor radionuclide therapy (PRRT) is an emerging modality for treating metastatic cancer. The current paradigm is to deliver a beta-emitting radionuclide to tumor sites via molecular specificity of the chelating agent.
The PRRT approach has shown efficacy in somatostatin-receptor positive cancers. The FDA has approved PRRT for treatment of metastatic midgut neuroendocrine tumors. The somatostatin analog octreotate with an attached “DOTA” chelator is known as DOTA-TATE, with (DOTA chelates the radionuclide). Imaging with gallium-68 (Ga-68) DOTA-TATE PET shows distribution of the cancer and subsequent therapy is accomplished with lutetium-177 (Lu-177) DOTA-TATE. Lu-177 is a beta emitter and the high energy electrons ablate the tumor from within. 
The image below shows that this approach can be used for other somatosttin receptor positive tumors. The patient has widely metastatic Merkel cell carcinoma.
