Patterns of disease control and survival in patients with melanoma brain metastases undergoing immune-checkpoint blockade

Limited data are available on the efficacy and clinical benefit in patients with melanoma brain metastases (MBMs). The aim of this study was to determine whether ICB is active in an unselected cohort treated of patients with known brain metastases and if disease control correlates with the survival.MethodsA total of 385 patients with metastatic malignant melanoma treated with ICB as monotherapy between 2005 and 2017 in two tertiary referral centres were included. Patient records were searched for the development of brain metastases. Demographic and clinical data of all patients were collected retrospectively.ResultsWe identified 177 patients with MBM who received ICBs (ipilimumab, nivolumab, pembrolizumab). Patients with and without brain metastases received similar ICB regimens. Prognosis was inferior in patients with brain metastases; patients with >1 brain metastasis showed even poorer survival. For extracranial (ec) metastases, disease control was associated with improved survival. However, when comparing patients with intracranial (ic) disease control during immunotherapy to patients with ic disease progression, no difference in OS could be observed.ConclusionsIn our study, ec disease control was the dominant predictive factor for OS in both patients with or without melanoma brain metastases. These data indicate that clinical trials in melanoma patients with brain metastases should address end-points such as symptom control, quality of life or OS in addition to ic response rates. https://ift.tt/2JBdKAM

Von Foto-Set bis Sport: Projektwoche am Evangelischen Gymnasium

Es waren mehr als ein Dutzend Gruppen - Schüler der Oberstufe des Evangelischen Gym­nasiums Hermannswer­der - die sich in einer Projektwoche mit unter­schiedlichsten Themen befassten. Dabei arbeite­ten sie wohlgemerkt nicht an einem Ober­thema - Vielfalt be­stimmte das Programm.

So kam es, dass sich zum Beispiel die Lauf­gruppe neben den For­schern der Gruppe „Ar­beit eines Historikers“ und die Schulchronisten Wand an Wand mit den Webdesignern wie­derfanden.

Die Aufgabe Letzterer war es, die Schulhomepage zu erneuern. Da­für schwirrten die Elft- bis 13.-Klässler mit Camcorder und Digital­kamera bewaffnet durch das Schulhaus, um die alten, verpixelten Bilder gegen neue, frische Ansichten auszu­tauschen.

Nebenan trafen sich die Chronisten mit ehemaligen Schülern und Lehrern, um die Schulgeschichte seit 1990 auf­zuarbeiten und den Webdesignern für die Intemetseite zur Verfügung zu stellen. Davon profitieren einige Schüler der Jahrgangsstufe 11, die wie Luise Milsch neu auf die Schule gekommen waren, be­sonders. Sie habe „gleich was lernen können und sich da­durch mehr zugehörig ge­fühlt“. Besonders interessant fand sie, „wie die Leute, vor al­lem die Lehrer, früher ausgese­hen haben“.

Im Keller trafen sich die Künstler, um „Film Stills“ zu erstellen, wie der etwas krypti­sche Projettitel verkündete. „Unsere Aufgabe ist es, Ge­schichten zu erzählen, die nicht eindeutig sind“, erläuter­ten die angehenden Abiturientinnen Julia Hirsch und Lydia Morgenstern'. „Das heißt, dass der Betrachter unserer Fotos sich die Erzählung im Kopf weiter denken kann“, so Ly­dia. „Wir haben uns ,Ein Tag im Leben der Bank’ zur Über­schrift gemacht und Fotos von immer derselben Bank ge­macht“, erklärt Julia. „Auf die­ser sitzen dann aber immer verschiedene Leute“. Auf Ba- sis dieses Konzepts folgte die Umsetzung - das Verkleiden. Das habe viel Spaß gemacht, so Julia lächelnd. Geachtet hät­ten die beiden bei ihren sieben Bildern nicht nur auf die Per­spektive, sondern auch „auf die Kontraste der Emotionen: Wir haben ein weinendes Mäd­chen, lachende Freundinnen oder auch ein verträumtes Ehe­paar“, so die 19-Jährige.

Mit Emotionen haben auch die Läufer zu tun, denn an­ders, als man annehmen könnte, sind die Mitglieder der Laufgruppe nicht die ganze Zeit gerannt, sondern haben sich auch gegenseitig in Vorträ­gen darüber informiert, wie man sich motiviert zu laufen.

Bei den Physikern wurde sehr viel und aufwendig ausge­wertet, nämlich die Experi­mentierergebnisse. Unter den Augen der Physiklehrer Dirk Lange und Peter Ackermann wurde das gesamte Experimentier-Equipment auf Herz und Nieren geprüft und alle nur erdenklichen Versuche durchgefuhrt. „Die Woche war so angelegt, dass alle Expe­rimentatoren frei die Experi­mente wählen konnten, die dann durchgeführt und deren Resultate schriftlich festgehal­ten wurden“, erklärt Dirk Lange. „Parallel dazu haben die Nicht-Experimentatoren, wir Lehrer, einige Demoversu- che wie das ,Fadenstrahlrohr’ aufgebaut.“

Die Woche endete dann mit einem Evaluationsfrühstück.

Cytomegalovirus reactivation in patients with refractory checkpoint inhibitor-induced colitis

While the majority of patients with irColitis respond well to corticosteroid treatment ± other immunomodulatory drugs such as infliximab, some patients do not show resolution of their symptoms. In the present study, we analysed the frequency of therapy-refractory irColitis, the underlying cause, and useful diagnostic approaches.MethodsBetween 2006 and 2016, 370 patients with metastatic malignant melanoma were treated with checkpoint inhibitors at the Department of Dermatology at the University Hospital Essen. All patients were identified for whom diarrhea and/or colitis was documented in the digital patient records. Patients who did not respond to standard immunosuppressive therapy within 2 weeks were classified as refractory. Demographic and clinical data of all patients were collected.ResultsWe identified 41 patients with irColitis, the majority occurring during treatment with ipilimumab. Amongst these, 5 (12.2%) were refractory to standard immunomodulatory treatment with corticosteroids and infliximab. Therapy-refractory cases tended to show more severe inflammation in colonic biopsies (p = 0.04). In all therapy-refractory cases cytomegalovirus (CMV) was detectable. CMV-DNA in colonic biopsies and in plasma was significantly more often detectable in therapy-refractory cases (in colonic biopsies p = 0.005, in plasma: p = 0.002). Presence of serum CMV IgM and positive immunohistochemical stainings of colon biopsies for CMV were also associated with refractory colitis (p=0.021; p = 0.053).ConclusionsThis report on CMV reactivation during management of checkpoint inhibitor-induced colitis emphasises the need for repetitive diagnostic measures in treatment-refractory irColitis. http://ift.tt/2h6NRbG
