Dear Friend of Ukraine, As we approach the end of another year, we want to take a moment to express our deepest gratitude for your unwavering support of United Help Ukraine. Your generosity has been instrumental in our efforts to provide critical aid and support to those affected by the ongoing war in Ukraine.
Thanks to donors like you, we have been able to make a real impact on the ground, from delivering essential supplies to supporting psychological initiatives for children affected by the war. In addition, we were able to bring wounded Ukrainian warriors with the most difficult amputations to receive highly specialized prosthetic treatment in the US.
Meet Gosha Roskha, 31, who is the father of two children, aged 7 and 5, and an amputee. But thanks to you, when he sees his children again, he will be able to embrace them with a new prosthetic arm. He is one of the ten defenders who United Help Ukraine has brought to the Washington, D.C. area for prosthetics and physical therapy with your help. Don't miss Gosha's powerful story on CNN! Hear firsthand about his experiences on the frontlines and his inspiring journey of receiving a prosthetic limb in the United States. Click HERE for an inspiring story of resilience and hope.
Seriously wounded on two separate occasions, Gosha lost his left arm as a result of a mortar blast and a tourniquet left on too long. He was among the survivors who refused to surrender until ordered at Azovstal, a steel plant in Mariupol. Gosha spent a month and a half in Russian captivity in horrible conditions. He was poorly fed, and his wound didn’t heal well. Rot set into the bone at the amputation site, near his left shoulder. Luckily, Gosha was freed in a prisoner exchange, unlike many other Mariupol defenders still remaining in Russian captivity.
Your support has not only enabled us to cover the journeys of heroes like Gosha, but has also served as a lifeline, offering a pathway for them to regain independence after selflessly sacrificing so much on the frontlines. We thank our partner organizations, Maryland Center for Orthotics and Prosthetics, Medstar Union Memorial Hospital, Revived Soldiers Ukraine, and Future for Ukraine, for working with us to transform the lives of these Ukrainian heroes. Additionally, we are tremendously grateful for the United Help Ukraine volunteers who act as drivers, interpreters, hosts, and often tour guides for the amputees as they recover from spiritual and psychological wounds.
“Gosha’s life is still challenging because he’s learning how to use the prosthesis, and it’s hard,” says UHU volunteer Regina Vasko, co-manager of UHU’s Wounded Warrior project along with Olena Bulgakova. “It will take a long time after his return to Ukraine.” Your donations power critical programs like these – from humanitarian aid to medical support and defender initiatives. A donation today ensures we can continue saving lives and aiding the resilient people of Ukraine.  For your end of year gift, stand with Ukraine by making a donation today. Thank you for being a part of our mission to help people and save lives in Ukraine.

I am TRYING to be fair to the "barefoot is best" people, because I know that emotionally I consider them as very similar to the "Go off your psych meds! Eat 16 oranges a day instead!" people. (Is that reference too dated now?)
I found a perfect demonstration of what I mean in my google searching. In a running forum, someone posted asking for help, since they have "significant hip and knee discomfort" and that "it seems that my foot has been the cause of the problem. The inner front of my left foot is raised, which causes me to roll in when I walk and run." This is the same issue I have! It's where the bare foot cannot rest fully on the ground without causing the ankle to collapse. The bare foot is intrinsically fucked up and cannot walk healthily without assistance.
The thread only has two replies:
Tumblr media
*JAZZ HANDS* BAD ADVICE EXTRAVAGANZA!
Because even as a podiatrist hawking his own invention admits, people like the OP whose feet chronically and dramatically under- or over-pronate, even if their toes are given every chance to work normally, should probably have orthotic assistance:
youtube
He says this is about 3% of the clients he sees, which is very comforting if you're marketing towards 97% of people and less comforting if you're not among them.
Something that enrages me is seeing people point to things like this 1949 study, "Survey in China and India of Feet That Have Never Worn Shoes". Its conclusion is more of a soundbite than any other scientific paper I have ever seen.
Conclusion
People who have never worn shoes acquire very few foot defects, most of which are painless and non-debilitating. The range of their foot motions are remarkably great, allowing for full foot activity. Shoes are not necessary for healthy feet and are the cause of most foot troubles. Children should not be encouraged to walk prematurely and should not wear any footgear until absolutely necessary. Footgear is the greatest enemy of the human foot.
Which is great... until you go back to the section about their sampling methods, which says. "Persons with any obvious generalized disease or deformity, acquired or congenital, were not considered as falling within the scope of the survey and so were not included."
(It's little things like that that remind you that in 1949, the Western medical establishment generally thought that disabled people ought to be segregated from the rest of society in asylums. One of my elementary school teachers had the goodness to recommend I read books by disabled Canadian author Jean Little, to give me a sense that I was not alone. Her first published novel was 1962's Mine for Keeps, about a girl with cerebral palsy who comes home to her family after living for five years at the Allendale School for Handicapped Children. She barely knows her siblings, and is absolutely amazed that her family has outfitted the house with things like carpets that won't slip out from under her crutches as she walks. She's the first person with cerebral palsy to ever attend her neighbourhood school. She gets friends and adopts a dog, it's adorable. But my point is, it's really useful to remember that when people say, "Back in the olden days, people didn't have these disabilities!" they're mostly unaware of the reality that back the olden days, people with those disabilities didn't get to go out in public.)
Spiders Georg is an outlier adn should not be counted... up until you are the doctor tasked with treating Spiders Georg! At which point it is not very helpful to say "Well, the average person eats 0 spiders a year, so I don't think spider consumption is something you need to worry about."
And if something affects 1% of the population, that means that in a statistically average city of 1 million people, ten thousand people have it.
So I'm still very "Oh fuck OFF" when someone wants to tell me that barefoot running worked for them and therefore shoes are bad for everybody. I'm even chronically stressed with places where I'm asked to take my shoes off at the door, because walking barefoot hurts and therefore I'm always having to decide between prioritizing my own health, and appearing rude and standing out as That One Weird Person With Her Shoes On. Especially since sometimes going barefoot has strong spiritual/religious symbolism, and I'd like to not be a walking desecration, you know?
But that doesn't mean the barefoot lobby is completely useless. I've had a lot of success learning from the scientists and doctors who study foot movement and have questioned standard podiatric treatment. I spent a long time getting blisters from orthotics that propped my foot into the correct position but didn't care at all about how my foot moved throughout the gait cycle.
I've spent ten years mostly away from podiatrists, learning what I can from pedorthists, physiotherapists, and ballet dancers. I've moved from using my right foot as a peg leg whose joints didn't articulate to being able to tiptoe and even, very cautiously, dance. I made myself a special 2" tall rubber mat so I can exercise barefoot while still giving my short leg the height boost it needs to put my hips even.
But being Spiders Georg is tiring as all fuck sometimes. Sometimes I'd like to be counted.

It turns out that watching wealthy women verbally attack each other at a Gucci brunch hits different when you’re sick. I’ve mostly spent the day sleeping and reading. I think I can rally now and get out of bed but definitely cannot stomach eggs Benedict. Maybe tomorrow.
I got an insurance company denial for Ava’s helmet. Apparently for babies under 6 months they have to do 2 months of repositioning before insurance will cover a helmet. She turns 6 months tomorrow 🙃. Also we’ve been repositioning her since the NICU so I’m not sure what “counts” as repositioning- maybe since it hasn’t been 2 months since her doctor referred us to orthotics (2 months from that would be 2/20). So I guess tomorrow I’ll call the office and request they resubmit everything all over again- what a waste of time. Couldn’t they just hold it and process it tomorrow? Also finally got an EOB for her speech therapy- it turns out the children’s hospital is in network (despite not coming up on the website) but the plan discount is way worse than what it was under the insurance company my employer had been using up until this year. Same billed amount but cost after discount is $358 with Cigna vs $202 with Aetna. Everyone is all up in arms about us switching insurance at work, and rightfully so.
I took work off Friday to avoid the daycare pickup rush when my husband works late on Fridays. Isla is off school so I’m going to take Ava to daycare so we can have some one on one time. She picked breakfast out over lunch and wants to go to the mall so that’s what we’ll do.

A giraffe born with a disorder gets modified leg braces designed for humans
ESCONDIDO, Calif. — Over the past three decades Ara Mirzaian has fitted braces for everyone from Paralympians to children with scoliosis. But Msituni was a patient like none other — a newborn giraffe.
The calf was born Feb. 1 at the San Diego Zoo Safari Park in Escondido, north of San Diego, with her front limb bending the wrong way. Zoo staff feared she could die if they didn't immediately correct the condition, which could prevent her from nursing and walking around the habitat.
But they had no experience with fitting a baby giraffe in a brace. That proved especially challenging given she was a 5-foot-10-inch-tall (178-centimeter) newborn and growing taller every day. So, they reached out to experts in orthotics at the Hanger Clinic, where Mirzaian landed his very first animal patient.
"It was pretty surreal when I first heard about it," Mirzaian told The Associated Press this week during a tour to meet Msituni, who was strutting alongside the other giraffes with no troubles. "Of course, all I did was go online and study giraffes for like 24/7 until we got out here."
Using cast moldings of the giraffe's legs, a carbon graphite brace features the animal's distinct pattern of crooked spots to match her fur. In the end, Msituni only needed one brace; and the other leg corrected itself with the medical grade brace.

Understanding Toe Walking: Causes, Treatment, and Prevention
Tumblr media
Toe walking, also known as equinus gait, is a condition where an individual predominantly walks on their toes, without placing their heels on the ground. While toe walking is common among toddlers during their early stages of walking development, it can persist or reoccur in older children and adults, causing discomfort and impairing walking patterns. In this article, we will explore the causes, treatment options, and preventive measures for toe walking.
Causes of Toe Walking:
There are several reasons why an individual may engage in toe walking:
Idiopathic Toe Walking: In most cases, the cause of toe walking is unknown. This is referred to as idiopathic toe walking and is often observed in young children. Idiopathic toe walking may be associated with muscle imbalances or neurological conditions.
Neurological Conditions: Toe walking can be a symptom of certain neurological conditions such as cerebral palsy, muscular dystrophy, or autism spectrum disorders. These conditions can affect muscle tone, coordination, and balance, leading to toe walking.
Tightness in the Achilles Tendon: Shortened or tight Achilles tendons can cause limitations in ankle movement, resulting in toe walking. This condition may be present at birth or can develop due to prolonged toe walking habits.
Treatment and Prevention:
Physical Therapy: Physical therapy plays a crucial role in managing toe walking. It helps to stretch and strengthen the muscles and tendons, improve range of motion, and correct abnormal walking patterns. Therapists may employ various techniques such as stretching exercises, gait training, and orthotic devices.
Orthotic Devices: In some cases, orthotic devices such as ankle-foot orthoses (AFOs) or shoe inserts can be beneficial in correcting toe walking. These devices provide support, alignment, and help maintain proper foot positioning while walking.
Casting or Bracing: In instances of severe toe walking or when other treatments fail, casting or bracing might be recommended. This involves wearing a cast or brace that gradually stretches the calf muscle and Achilles tendon, aiding in correcting the walking pattern. Read more here: https://ankleandfootcenters.com/bunion-removal/.
Preventive Measures:
While it may not be possible to prevent all instances of toe walking, the following measures can help reduce the risk:
Early Intervention: Identifying and addressing any underlying causes of toe walking in infancy or early childhood can help prevent the persistence of this condition into later stages of life. Regular check-ups with pediatricians and monitoring motor development can be beneficial.
Encouraging Proper Footwear: Wearing appropriate-sized shoes with proper arch support and cushioning can help maintain proper foot positioning and reduce the likelihood of toe walking.
Promoting Healthy Foot and Leg Muscles: Encouraging children to engage in activities that promote overall muscle strength and flexibility can contribute to proper walking patterns and reduce the risk of toe walking.
In conclusion, toe walking is a condition where an individual habitually walks on their toes. While it may be common in young children, persistent or reoccurring toe walking can be a cause for concern. Understanding the causes, seeking appropriate treatment, and implementing preventive measures can help address toe walking and improve walking patterns. ‘We saved a giraffe’s life’: calf fitted with braces to correct bent legs
Msituni was born with her front limb bending the wrong way, and her other front limb started to hyperextend
Associated Press
Fri 13 May 2022 18.10 (CEST)
Over the past three decades Ara Mirzaian has fitted braces for everyone from Paralympians to children with scoliosis. But Msituni was a patient like none other: a newborn giraffe.
The calf was born 1 February at the San Diego Zoo Safari Park in Escondido, north of San Diego, with her front limb bending the wrong way. Safari park staff feared she could die if they didn’t immediately correct the condition, which could prevent her from nursing and walking around the habitat.
But they had no experience with fitting a baby giraffe in a brace. That proved especially challenging given she was a 5ft 10in (178cm) newborn and growing taller every day. So, they reached out to experts in orthotics at the Hanger Clinic, where Mirzaian landed his very first animal patient.
“It was pretty surreal when I first heard about it,” Mirzaian told the Associated Press this week during a tour to meet Msituni, who was strutting alongside the other giraffes with no troubles. “Of course, all I did was go online and study giraffes for like 24/7 until we got out here.”
Zoos increasingly are turning to medical professionals who treat people to find solutions for ailing animals. The collaboration has been especially helpful in the field of prosthetics and orthotics. Earlier this year, ZooTampa in Florida teamed up with similar experts to successfully replace the beak of a cancer-stricken great hornbill bird with a 3D-printed prosthetic.
Tumblr media
Msituni is seen at the San Diego Zoo Safari Park in February 2022. Photograph: Ken Bohn/AP
The Hanger team in California had fit orthotics for a cyclist and kayaker who both went on to win medals at the 2016 Paralympics in Brazil and customized a brace for a marathoner with multiple sclerosis who raced in seven continents.
And in 2006, a Hanger team in Florida created a prosthetic for a bottlenose dolphin that had lost its tail after becoming tangled in ropes from a crab trap. Their story inspired the 2011 movie Dolphin Tale.
But this was a definite learning curve for all, including Matt Kinney, a senior veterinarian for the San Diego Zoo Wildlife Alliance in charge of Msituni’s case.
“We commonly put on casts and bandages and stuff. But something that extensive, like this brace that she was provided, that’s something we really had to turn to our human [medicine] colleagues for,” Kinney said.
Msituni suffered from hyperextended carpi, wrist joint bones in giraffes’ front limbs, which are more like arms. As she overcompensated, the second front limb started to hyperextend as well. Her back leg joints also were weak but were able to be corrected with specialized hoof extenders. And given that she weighed more than 100lb (55 kilos) at birth, the abnormality was already taking its toll on her joints and bones.
While the custom braces were being built, Kinney first bought post-surgery knee braces at Target that he cut up and re-sewed, but they kept slipping off. Then Msituni wore medical grade braces for humans that were modified for her long legs. But eventually Msituni broke one.
For the custom braces to work, they would need to have a range of motion but be durable, so Hanger worked with a company that makes horse braces.
Using cast moldings of the giraffe’s legs, it took eight days to make the carbon graphite braces that featured the animal’s distinct pattern of crooked spots to match her fur.
“We put on the giraffe pattern just to make it fun,” Mirzaian said. “We do this with kids all the time. They get to pick super-heroes, or their favorite team and we imprint it on their bracing. So why not do it with a giraffe?”
In the end, Msituni only needed one brace. The other leg corrected itself with the medical grade brace.
When they put her under to fit the custom brace, Mirzaian was so moved by the animal’s beauty, he gave her a hug.
“It was just amazing seeing such a big, beautiful creature just laying there in front of me,” he said.
After 10 days in the custom brace, the problem was corrected.
All told, she was in braces for 39 days from the day she was born. She stayed in the animal hospital the entire time. After that, she was slowly introduced to her mom and others in the herd. Her mom never took her back, but another female giraffe has adopted her, so to speak, and she now runs along like the other giraffes.
Mirzaian hopes to hang up a picture of the baby giraffe in her patterned brace so the kids he treats will be inspired to wear theirs.
“It was the coolest thing to see an animal like that walk in a brace,” he said. “It feels good to know we saved a giraffe’s life.”

Common Pediatric Orthopedic Conditions:
1. Developmental Hip Dysplasia (DDH): DDH involves hip joint abnormalities, requiring early screening and treatment like braces or surgery for severe cases, particularly common in females and breech-positioned infants.
2. Clubfoot (Congenital Talipes Equinovarus): This condition, affecting 1 in 1000 births, demands personalized treatment plans, ranging from casting to corrective surgeries, to address inwardly twisted feet and tight Achilles tendons.
3. Scoliosis: Vigilant monitoring and intervention, including bracing or surgery for moderate to severe cases, are essential to manage lateral spine curvature during adolescence and mitigate associated symptoms.
4. Osgood-Schlatter Disease: Characterized by knee pain in active adolescents, this condition necessitates conservative measures like rest and physical therapy to alleviate symptoms and prevent recurrence.
5. Pediatric Fractures: Children's fractures require appropriate immobilization and follow-up care to ensure optimal healing and minimize long-term complications, with surgery reserved for complex cases.
6. Neuromuscular Conditions: Disorders like cerebral palsy and muscular dystrophy require multidisciplinary management to address musculoskeletal abnormalities and optimize mobility through tailored interventions.
7. Limb Length Discrepancy (LLD): LLD may be managed conservatively with orthotic devices for mild cases or surgical procedures for significant discrepancies to achieve functional harmony.
8. Pediatric Sports Injuries: Prevention strategies and prompt management of sports-related injuries are crucial for minimizing downtime and preventing long-term complications in children and adolescents.
9. Genetic Disorders: Conditions like Marfan syndrome and osteogenesis imperfecta require a multidisciplinary approach to address musculoskeletal manifestations and associated complications through various treatment modalities.
Orthopedic conditions can be detected and treated early-on in children by opting for a regular full body health checkup for children at a good hospital.

Baby Helmets Are Becoming More Common - but Are They Really Necessary?
Morrison Goodwin was born in March 2018 with a head that looked a bit like a "Lego block," says his mom Georgina, a sales executive in Los Angeles. Morrison was twin baby A, packed below his sister Rae at the bottom of Goodwin's uterus. Goodwin mentioned her observation about her son's head to her pediatrician, who recommended various exercises and repositioning with the help of their physical therapist. When Morrison's head shape didn't budge, their pediatrician suggested visiting a cranial remolding orthosis clinic. The doctors there diagnosed him with moderate plagiocephaly, suggesting it might eventually impact his sleep and bite, and recommended a baby helmet to round out his head. "We were very skeptical. It wasn't a slam dunk," Goodwin says. "This is a business. I'm guessing that they're more likely to want to give you a helmet, if that's how they make their money." Even so, Goodwin warily gave the green light. "Our insurance paid for it. And it's very noninvasive. You're a new parent. You're like, well, I'm supposed to set this kid up for success, and I'm told to do this," she tells POPSUGAR. Plagiocephaly and brachycephaly - two types of flat head syndromes - have recently been on the rise in the United States. In 1994, the National Institutes of Health launched its Back to Sleep campaign, after a slew of studies showed that tummy sleep is linked to an increased risk of sudden infant death syndrome (SIDS). "It worked wonderfully," says John Girotto, MD, FAAP, FACS, the section chief of pediatric plastic surgery at Helen DeVos Children's Hospital in Grand Rapids, MI, who specializes in craniofacial surgery. Rates of SIDS dropped from 130 to 78 deaths per 100,000 live births between 1990 and 1996. "But what happened is kids developed misshapen skulls," he adds. Because babies now spend more time lying on their backs, their soft heads develop flat spots. At first, many pediatricians and surgeons were concerned about the long-term effects, Dr. Girotto says. But by the late 1990s, researchers confirmed that babies simply had "positional" molding and most didn't require surgery. Today, however, an entire industry promises to fix babies' flat spots via "helmet therapy," also called helmet orthosis. Clinics with mottos like "reshaping children's lives" devise their own guidelines for helmet therapy and sell baby helmets, which can be personalized with Star Wars and Minnie Mouse motifs. Some have Instagram accounts with tens of thousands of followers. Got an especially photogenic baby? You can even submit a pic of your helmeted baby to your orthotic company's "calendar contest." To be clear, these clinics probably aren't harming their patients. But the question of how often baby helmets are really necessary remains. Amy Mischnick, PT, a pediatric physical therapist at Cincinnati Children's Hospital who treats flat head syndrome, says that part of her job is to provide evidence-based recommendations to parents. This includes, she says, correcting "misinformation, especially when it is used as a scare tactic to place emotional pressure on parents to pursue helmeting not based on facts but on unfounded fear and guilt." Why Do Babies Get Flat Heads, Anyway? Some babies, like Morrison, are born with a flat spot (aka positional skull deformity), often due to their position in the womb. But most flat spots tend to develop gradually after birth, due to pressure from staying in the same position while sleeping and playing. The most common positional skull deformity is plagiocephaly, a flat spot on one side of the head that makes the opposite forehead prominent, which may affect nearly half of all 4-month-old babies. Brachycephaly is less common and occurs when the back of a baby's skull is very flat and wide. Parents usually notice a flat spot within three months of birth, when a baby's skull is soft and moldable. "At that point in time, behavior modifications usually do a great job of helping the skull round… https://www.popsugar.com/family/baby-helmets-flat-heads-49344030?utm_source=dlvr.it&utm_medium=tumblr
