Izuku gets a quirk in order to survive the collateral damage of a villain fight as a kid, it's an animal shifting quirk but he is a little kid so he doesn't know how to shift back. In his fear from the fight still going on he hops away (lil bunny) and hides in a park where a cat with an animal quirk finds him.
The cat cares for him as their own and carries him around by the scruff when they have to climb or jump where a little bunny can't go. One of those places happens to be Aizawa's apartment balcony where he feeds the stray cats.

Ochako has had it with being neglected in her relationship with Izuku. He's always away on business, working too much, or meeting other heroes. He has no time for her and absent-mindedly ignores her. Aizawa takes on a case involving a cold missing persons case because he is free from teaching that year (the year he expelled his whole class). He works hard to find and gain the teen's trust, no one knows who he is. Aizawa learns over months that the teen can barely talk from years alone and is more animal then person currently, luckily he is more curious then scared of Aizawa, and he can be convinced to show himself for a jelly packet Aizawa likes to have on him.
Eventually he gets enough trust to be shown the teen's home, an abandoned RV that got washed out from a campsite during a storm years ago, he finds the possessions the teen had from years ago. The little worn out red toddler shoes hurt to see first, it tells him how long Izuku has been out here, and some 'Eraserhead' style kids sunglasses show why he trusts Aizawa more than anyone else.
Reading the notebooks and seeing a scared quirkless child grow to be a little confident in surviving to feral over time is the most heartbreaking thing Aizawa has had to deal with as an underground hero.
